Complete prescription form for each case. We provide forms at no charge and can also be downloaded here. Prescription forms can also be accessed from the DDDL portal. STL files can also be sent through our customer portal, or emailed to digitalscans@digitaldesignsdl.com.

Securely box up case contents. Place case contents and completed Rx form inside box and close. Seal box with tape.

• Shipping tips to ensure safe delivery:
  • Wrap each model individually with bubble wrap or foam padding. This will ensure your models and other case materials are not damaged during shipment (particularly important for any wax up). Additional bubble wrap should be added to fill empty space and help avoid movement/breakage during shipping.
  • When shipping an articulator and face-bow:
    • Remove the models from articulator and wrap individually
    • If models remain on the articulator, please separate the articulator. Each piece should be wrapped individually and anchored in the box. Please wrap face-bow and other enclosures separately.
  • Shipping any type of alginate: Please put Rx in a separate sealed plastic bag to avoid getting wet and compromised. Staple the Rx bag to appropriate case bag.
  • Shipping loose crowns, bridges, implants parts and/or shade tabs: Place in a separate small container or sealed plastic bag to avoid loss or breakage and staple to Rx. Please be sure to include the patient’s name and staple to the Rx.
  • Photos that accompany the case should be placed in a plastic bag for protection.
  • To ensure that cases are not mixed up, if shipping multiple cases in one box, please bag each case separately.
